Home Assessment

Comprehensive evaluation in your home to determine if modifications and/or equipment is necessary to maximize your independence or assist in your personal care. The evaluation may include recommendations for ramp construction, bathroom and kitchen modifications/equipment.

Functional Assessment

Comprehensive evaluation of your level of functioning in areas of self-care, homemaking and work skills to determine if greater independence can be achieved with adaptive equipment, training in work simplification/safety techniques or other specific Occupational Therapy treatment.

Wheelchair Assessment

Comprehensive evaluation of your mobility needs in the home and community to determine if a wheelchair (manual or powered)/scooter would allow you to be more independent or to participate in more activities of daily living.

Driving Assessment

Comprehensive assessment of a person's physical, visual and cognitive skills to determine if you can drive safely. This includes a vehicle, adaptive equipment, in-traffic assessment.

Follow Up Services

- Thorough written documentation of all findings and recommendations
- Assistance with equipment or modification purchase and installation
- Family and client education
- Final inspection of the modifications and/or equipment to insure follow through with recommendations and proper fitting to individual
- Driver Training/Education Course
